It's easy to use your Health Discounts membership benefit. Once you have signed up and received your information kit, simply follow these directions and start using your ID card today! (Some discounts will have specific directions on how to use. Please review your information kit for details.)
STEP 1 |
Keep your ID card with you at all times so it will be easily accessible to present at provider locations. You must present your ID card at the time of service to receive a discount. |
STEP 2 |
To locate and verify participating providers in your area, simply call Customer Service toll-free at (844) 279-3195. When calling to schedule an appointment, please refer to your ID card for proper network identification when speaking to a participating provider's office. |
STEP 3 |
You are responsible for paying the provider the entire discounted fee at the time the service is provided. |
