Once you have joined the plan and recieved your membership kit, simply follow these directions and start using your membership card today!

1. Keep your membership card with you at all times so it will be easily accessible to present at provider locations. You must present your membership card at the time of service to receive a discount.
2. To locate and verify participating providers in your area, simply call Careington's Member Services at (800) 290-0523. You may also visit www.careington.com to search for providers. When calling to schedule an appointment, please refer back to the membership card for proper network identification when speaking to a participating provider’s office.
3. You are responsible for paying the provider the entire discounted fee at the time service is provided.
4. If you have any questions about how to use your discount plan, call Careington's Member Services department toll-free (800) 290-0523, anytime between 7 a.m. and 7 p.m. CST, for prompt and friendly assistance.

THIS PLAN IS NOT INSURANCE and is not intended to replace health insurance. This plan does not meet the minimum creditable coverage requirements under M.G.L. c.111M and 956 CMR 5.00. This plan is not a Qualified Health Plan under the Affordable Care Act. This is not a Medicare prescription drug plan. The range of discounts will vary depending on the type of provider and service. The plan does not pay providers directly. Plan members must pay for all services but will receive a discount from participating providers. The list of participating providers is on this website. A written list of participating providers is available upon request. You may cancel within the first 30 days after effective date or receipt of membership materials (whichever is later) and receive a full refund, less a nominal processing fee (nominal fee for MD residents is $5, AR residents will be refunded the processing fee).
