| Members can log in to create a custom tailored provider search based on your individual or group plan. You may login using either your member id or your social security number. Your member id may be obtained from your CAREington International Dental Card, or appropriate benefit card. If you don't know your member number, please call Member Services at 800-290-0523. |

Performing a search: |

In order to create a customized provider directory listing,
The Distance in Miles and either a Zipcode or a City
and State are required. The search feature will return
providers within the area you specify. By changing the search options,
your listing will contain the providers matching your criteria.
To search for a specific last name, enter all or part of the
providers last name. For example, If you wanted to find all providers with the last name Smith who are Oral Surgeons within 10 miles of the zipcode 75248, then you would select < 10 miles in the Select Distance in Miles field, Oral Surgeons in the Specialty field, 75248 in the Zipcode field, and Sm in the Providers Last Name field. |

Results Page Help: |
| The list of providers are displayed based on the criteria specified on the Provider Search Page. A maximum of 25 providers will be displayed per search. This list is displayed for viewing on a monitor. If you would like to print a copy, it is recommended that you click the Printable Page button on the top or bottom of the page. This will properly display the providers so that the printing will not be disrupted by page breaks. If you would like to edit your search criteria you may click the Search Again button which will return you to the Provider Search Page with the previously selected option already selected. |
