(PHCS)
Private Health Care Systems offers a provider network of over
350,000 doctors at over 750,000 locations with discounts ranging from 20% – 45%
savings and an average of 40% savings*. PHCS is the first and only national PPO
to earn four endorsements of quality from two independent, nationally
recognized quality assurance organizations, the National Committee for Quality

Caremark provides discounted prescription services with
significant savings for members. With over 57,000 participating pharmacies,

the nation. Members receive immediate access to an average savings of 31% on generic drugs and an average savings of 13% on brand name prescriptions. Members can order

About VSP Access Plan
The VSP program offers its members
substantial discounts on corrective eyewear, exams and surgical procedures
including LASIK surgery. Members receive 20% discounts on eye exams, frames and
lenses. Members also receive a 15% discount on contact lens services. Members
have access to nearly 30,000 eye care professionals, many of them local
providers, nationwide. Most frames, lenses, and specialty items such as tints,
scratch resistant coatings and ultraviolet protection are
available.
About EyeMed Vision Care Access Plan D
With more than 40,000 network providers, EyeMed Vision Care
Access Plan D gives members access to a provider panel of
optometrists, opticians and retailers, among them: LensCrafters®, Sears®, Pearle
Vision® and other well-known retailers. The network offers savings on vision
discounts, including corrective eyewear, examinations, and some surgical
procedures. Discounts on eyeglasses, contact lenses and other retail eyewear
