The following insurance benefits are provided as part of your membership.

Accidental Death & Dismemberment – Up to $15,000
Pays benefits up to the coverage amount for death or a covered dismemberment resulting from a covered accident.

Accident Medical/Dental Expense Benefit – Up to $5,000 per occurrence
Pays for covered medical expenses resulting from a covered accident up to the maximum benefit amount per covered incident. Subject to a $100 deductible.

All members of the Program are enrolled into the U.S. Benefits Association to be eligible to receive these insurance benefits. New York State with their principal place of business located at 830 Third Avenue NY, NY 10022-6565, licensed in all states, plus DC. These benefits are provided under the Group AD&D and Medical Care Insurance Policy (Form No. G-19000) issued to the U.S. Benefits Association and are subject to the exclusions, limitations, terms and conditions of coverage as set forth in the Certificate of Insurance provided in your membership materials. The insurance premiums for these benefits are paid by Careington out of membership fees collected. The membership fees charged for this program are not insurance premiums. This is not basic, comprehensive or major medical insurance plan and should not be used as a replacement for such insurance.
